Rule 781-19.5 - Filing of petition

A petition for a waiver must be submitted in writing to the agency as follows:

(1)License or authorization application. If the petition relates to a license or authorization application, the petition shall be made in accordance with the filing requirements for the license or authorization in question.

(2)Contested cases. If the petition relates to a pending contested case, the petition shall be filed in the contested case proceeding, using the caption of the contested case.

(3)Other If the petition does not relate to a license application or a pending contested case, the petition shall be submitted in writing to the treasurer of state's office.
