Sec. 99.
"Transition planning" means the process of transitioning a child from Part C (early intervention) to Part B (early childhood) that occurs prior to the child's third birthday to enable the child to experience a smooth and effective transition. The transition planning process is initiated by the Part C service coordinator, bringing together public agencies and the family to plan a process that maximizes continuity and minimizes disruptions in services.
