Section 470 IAC 3-4.7-40 - Children's medication records

Sec. 40.

(a) Records regarding dispensing of medication to children shall be maintained at the child care center and shall contain the following:

(1) The child's name.

(2) The name of the medication.

(3) The dosage given.

(4) The date and time given.

(5) The full name of caregivers who administered medication.

(b) If a parent requested medication to be given and it was not administered, caregivers shall write the reason on the medication record.

(c) The center shall keep these records for a minimum of one (1) year.
