Sec. 36.
(a) The child caring institution shall report the following changes or events to the SDPW prior to occurrence:(1) Change in licensed capacity.(2) Major alterations or changes in buildings or in use of rooms.(3) Addition or termination of program services offered.(4) Changes in administrative personnel.(5) Termination of services.(6) Changes in discipline policies.(7) Changes in confinement room policies, if applicable.(b) The child caring institution shall report the following changes or events immediately upon occurrence: (1) A fire on the premises of the child caring institution.(2) Death or serious injury requiring treatment of a child in a hospital or emergency care facility.(3) Any communicable disease requiring hospitalization of a child.(4) Any suspected or known incidents or evidence of child abuse or neglect. Such a report does not replace any other duty as required under IC 31-6-11 [IC 31-6 was repealed by P.L. 1-1997, SECTION 157, effective July 1, 1997.].(5) A court ordered placement that results in an excess in the number of children authorized by the license.
