Section 16.03.19.114 - VOLUNTARY CLOSUREWhen choosing to voluntarily close a CFH, the provider must give written notice at least thirty (30) days in advance to the residents, or the residents' representatives when applicable, and the certifying agent in the region where the home is located. The notification must include the following: (3-28-23)
01.Date of Notification. (3-28-23)02.Provider's Certificate. A copy of the certificate, or information from the certificate that includes: (3-28-23) a. The provider's name; and (3-28-23)b. Address of the home; or (3-28-23)c. Certificate number. (3-28-23)03. Closure Date. The written notice must include the planned closure date. The Department will not refund or prorate prepaid certification fees on retroactive closures. (3-28-23)04.Discharge Plans.
