Rule 375-5-1-.08 - Changes of Officers or Address(1) The Department must be notified in writing within then (10) days of any change of address of the owner, partner, or officer of any driver improvement clinic.(2) The Department must be notified in writing within ten (10) days of any change in officers, directors, or controlling shareholders of any corporation or association holding a license. In such case, each new officer, director or controlling stockholder must provide to the Department the same information as required for the original license application.(3) Failure to notify the Department of such changes shall be grounds for immediate suspension of the clinic license or the imposition of a civil penalty.Ga.
