Section 36a-1-66 - Forms and instructions(a) Copies of forms and instructions for the preparation of requests or applications for certain approvals, authorizations or licenses are available on the department's web site, the Connecticut Licensing Info Center web site at http://www.ct-clic.com or may be obtained upon request from the office of the commissioner.(b) The commissioner may from time to time adopt additional forms and instructions, and alter, amend or discontinue any form or instruction.Conn.
