Section 19-13-D70 - Contracted servicesHome health care agencies may hire other organizations, agencies or individuals to provide services to home health care agency patients. Services provided by the primary agency through arrangements with a contractor agency or individuals shall be set forth in a written contract which clearly specifies:
(a) That the patient's contract for care is with the primary agency;(b) The services to be provided by the contractor;(c) The necessity to conform to all applicable primary agency policies, including personnel qualifications, supervisory ratios and staffing patterns;(d) The responsibility for participating in developing the patient care plans;(e) The procedures for submitting clinical and progress notes, scheduling visits, periodic patient evaluation, and determining charges and reimbursement;(f) The procedure for annual assurance of clinical competence of all personnel utilized under contract;(g) A term not to exceed one year.Conn.
