The Department shall issue license documents only after the applicant has successfully complied with the procedure set forth in 333 CMR 9.05. The license documents shall include, at least, the following information: name; address; license number; any limitations or restrictions; the departmental validation and the applicant's signature. The document shall be available at the point of sale.
