Section 3.05 - Responsibilities of Certified Community Health Workers to the Board(1)Change of Personal Data. A Certified Community Health Worker shall provide written notice to the Board, in a form and manner prescribed by the Board, within 30 days of any changes to his or her: (b) address of record; or(c) social security number.(2)Responses to Board Requests for Information. Except as otherwise provided by law, a Certified Community Health Worker shall provide timely responses to Board requests for information relating to: (a) any application he or she has filed with the Board; or(b) any investigation or formal complaint pending before the Board pursuant to 272 CMR 9.02: Investigation and Initiation of Formal Complaints.(3)Reporting Actions by Other Licensing Authority. A Certified Community Health Worker shall provide written notice to the Board within 30 days of any action taken by a licensing or authorizing agency, within or outside the Commonwealth, against any certificate, registration or license which he or she may hold that authorizes healthcare related practice.Adopted by Mass Register Issue 1368, eff. 6/29/2018.Amended by Mass Register Issue 1377, eff. 6/29/2018.
