The Department shall utilize a standard form of Voluntary Placement Agreement, as established by the Department. This Voluntary Placement Agreement shall automatically expire after six months, and must be re-executed if the voluntary placement is to be continued. All Voluntary Placement Agreements shall be approved by, and then signed by, a departmental Area Director or Assistant Area Director or his/her designee. The Voluntary Placement Agreement is intended to be a flexible document adaptable to the individual needs and circumstances of the client or family thus the standard form may be modified as appropriate, so long as any such modifications are in writing and are approved by both the client and the Department.
