Section 76225 - Report of Changes(a) The licensee shall, within 10 days, notify the Department and shall, upon forms obtained from the Department, provide all information and documentation of any change in the information provided to satisfy the provisions of Section 76205.(b) Each licensee shall inform the Department within 10 days in writing of any change of the mailing address of the licensee. Such writing shall include the new mailing address of the licensee.(c) When a change in the principal officer of a corporate licensee (chairman, president, administrator or general manager) occurs the Department shall be notified within 10 days in writing by the licensee. Such writing shall include the name and business address of such officer.(d) Any decrease in licensed bed capacity, or a change of name of the facility shall require notification by letter to the Department, and shall result in the issuance of a corrected license.Cal.
