Section 3170.1 - Citation FormatEach citation:
(b) shall describe the nature of the violation, including specific reference to the provision of law determined to have been violated;(c) may contain an assessment of an administrative fine, an order of abatement fixing a reasonable period of time for abatement of the violation, or both an administrative fine and an order of abatement;(d) shall inform the cited person that, if he or she desires an informal conference to contest the finding of a violation, the informal conference shall be requested by written notice to the board within 10 calendar days of service of the citation;(e) shall inform the cited person that, if he or she desires a hearing to contest the finding of a violation, that hearing shall be requested by written notice to the board within 30 calendar days of service of the citation as provided for in subsection (b)(4) of Section 125.9 of the Business and Professions Code;(f) shall be served upon the cited person personally or by certified mail, return receipt requested;(g) shall inform the licensee that failure to pay a fine within 30 days of the date of assessment, unless the citation is being appealed, may result in disciplinary action being taken by the board. If a citation is not contested and a fine is not paid, the full amount of the assessed fine shall be added to the fee for renewal of the license.Cal. Code Regs. Tit. 16, § 3170.1
