Incident: Severe Storms, Tornadoes, Straight-line Winds, and Flooding.

Incident Period: 1/21/2017 through 1/22/2017.

Effective Date: 2/15/2017.

Physical Loan Application Deadline Date: 4/10/2017.

Economic Injury (EIDL) Loan Application Deadline Date: 11/07/2017.

All other information in the original declaration remains unchanged.
