The Treasurer of a Committee shall keep an account of:
(a) All contributions, gifts, and donations made by or on behalf of such committee.
(b) The name, address, voter’s identification number or driver’s license number of any person who makes contributions, gifts or donations of two hundred dollars ($200) or more;
(c) The name, address, voter’s identification number or driver’s license number of any person who makes contributions, gifts or donations that exceed one thousand dollars ($1,000) annually, as well as the date and amount of such contributions, gifts, or donations.
(d) The name and address of any political action committee that makes contributions, gifts or donations, as well as the amount and date of such contributions.
(e) The name and address of any person to whom a disbursement is made, as well as the date, amount, and purpose thereof, and as appropriate, the name of the party, candidate or aspirant on whose benefit the disbursement was made, and the office sought by the candidate or aspirant. The treasurer shall also keep a receipt, invoice or cancelled check for every disbursement of two hundred dollars ($200) or more.
(f) The funds of all political action committees shall be kept segregated and shall not be mixed with the funds of any person.
