The Board shall charge the following fees:
(1) | Application fee: | $110 | ||
(2) | Biennial license renewal: | |||
(a) | physical therapist: | $80 | ||
(b) | physical therapist assistant: | $70 | ||
(3) | Late Renewal Processing Fee: | $150 | ||
(4) | Deactivation: | $50 | ||
(5) | Reactivation (inactive to active): | $150 + renewal fee | ||
(6) | Reinstatement (lapsed to active): | $300 + renewal fee | ||
(7) | Miscellaneous Fees | |||
(a) Name Change and New License | $10 | |||
(b) Duplicate License | $10 | |||
(c) Duplicate Certificate | $10 | |||
