Section 15.153 - Contents of reports(a) Written reports under §§ 15.151 and 15.152 (relating to general requirements; and additional reporting requirements) shall be made on forms supplied or approved by the Department.(b) The report shall include, at a minimum, the following information: (1) Name, age and address of recipient.(2) Name, address of recipient's guardian or next-of-kin.(3) Facility name and address.(4) Description of the incident.(5) Specific comments or observations.The provisions of this §15.153 adopted May 17, 2002, effective 5/18/2002, 32 Pa.B. 2412.
