Section 06C .0337 - BASIC ENTITY DATA TO APPLY FOR A NORTH CAROLINA EDUCATOR LICENSE

All applicants for a North Carolina educator license shall provide the following basic entity data through the North Carolina Department of Public Instruction's secure, online licensure system:

(1) Social Security Number;

(2) Full legal name;

(3) Date of birth;

(4) Mailing address;

(5) Telephone number; and

(6) Non-school email address.
