Section R. 299.2455 - Notice of serious accident; reportingRule 2455.
(1) A person shall notify the supervisor of mineral wells or authorized representative of the supervisor of mineral wells of a serious accident that has caused or may cause waste. The notification shall be made by telephone as soon as possible but no more than 8 hours after discovery of the accident, and shall give the particulars of the accident. A detailed written report shall be submitted to the supervisor of mineral wells or authorized representative of the supervisor of mineral wells within 15 days of the accident.(2) If a person cannot contact the supervisor of mineral wells or authorized representative of the supervisor of mineral wells after an accident, then the person shall immediately telephone the pollution emergency alerting system.Mich.
