Rule 157-5-.02 - Documentation Requirements(1) A licensee must retain all required supporting documents as proof of completion of continuing professional education for no less than five (5) years.(2) Proof of continuing professional education may be documented by:(a) A copy of the licensee's current Commission on Dietetic Registration (CDR) card and CDR record of continuing professional education; provided, however, that the continuing professional education complies with the requirements specified within this chapter; or(b) Certificate of attendance including date of event, number of contact hours, provider and provider contact information and a program title and agenda or description of the course; or(c) Official college or university transcripts and a brief course description of academic coursework; or(d) Other documentation or appropriate reporting form(s) as determined by the Board.(3) Board Continuing Education Report Form must be provided with the above documentation if selected for a CE Audit at renewal.Ga.
