(a) "Speech pathology and/or audiology services" means those services referred or ordered by a licensed healthcare practitioner acting within the scope of his or her professional licensure or certification, for the provision of diagnostic screening and preventive and corrective therapy for persons with speech, hearing and/or language disorders.(b) Speech pathology and/or audiology service shall include but not be limited to the following: (1) Conducting and preparing written initial and continuing assessment of a patient.(2) Notes written and entered in the patient's health record after each treatment. The notes shall indicate the treatment performed, the reaction of the patient to the treatment, and be signed by the speech pathologist or audiologist.(3) Instruction of other health team personnel and family members in methods of assisting the patient to improve or correct a speech or hearing disorder.(c) A speech pathology and/or audiology service unit shall meet the following requirements: (1) Patient health records shall contain a patient's history and signed orders for treatment.(2) Progress notes shall be written at least weekly and entered in the patient health record and shall be signed by the speech pathologist and/or audiologist.Cal.
