PETITION TO AUTHORTIZE UTILIZATION OF MEDICAL MARIJUANA & PHYSICAL FITNESS FACILITY (REHAB) AS REQUESTED BY PRIMARY CARE PHYSICIAN
AND NOW, comes the defendant, Ories R. Smith, Pro Se, who respectfully requests that this United States Honorable Court grant the within PETITION TO AUTHORTIZE UTILIZATION OF MEDICAL MARIJUANA & A DESIGNATED PHYSICAL FITNESS FACILITY (REHAB) AS REQUESTED BY PRIMARY CARE PHYSICIAN, and in support thereof, states the following:
1. This United States District Court is presently supervising Ories Smith on a pretrial release, pending trial at case 2:21-CR-00482-MJH
2. Per Criminal Complaint, Ories Smith was Shot multiple times on June 29, 2021, and transported to the hospital in critical condition. Following being detained at the hospital after surgery, Mr. Smith was transported directly to the Allegheny County jail in an unstable mental and physical condition.
3. Mr. Smith was housed in the infirmary for 30 days for extensive wound care on 2 3-hour lockdown without seeing any family members after the incident on June 29, 2021, which was detrimental to his mental health following trauma.
4. The Allegheny County jail offered minimal to no physical therapy or mental health therapy for Mr. Smith to recover.
5. The Allegheny County jail Mental Health department Evaluated Mr. Smith late November and Diagnosed him with Chronic Anxiety and Sever Post Traumatic Stress Disorder (PTSD) suffering from Night terrors and reoccurring flashbacks of the event that took place on June 29, 2021
6. Mr. Smith still has multiple bullets lodged in his body and is in need of everyday physical fitness and rehabilitation.
7. Mr. Smith Has met with his doctor Emma Foy, at Premier Medical Associates Under Allegheny Health Network and has the recommendation to receive his medical marijuana card also to do physical fitness/Rehabilitation at least 5 days a week, considering his mental and physical diagnosis
8. Mr. Smith Provides this Honorable Court with the following documentation to support the claims above:
