ORDER

RONNIE ABRAMS, UNITED STATES DISTRICT JUDGE

On November 16, 2021, the Court received a “motion for obtaining compensation” from Defendant Jason Mojica in connection with “pain & suffering, and mental anguish, extreme excessive force . . . [and] aggravated sexual assault, ” which he claims to have suffered at the New York Metropolitan Correctional Center. Dkt. 65. If Defendant wishes to bring a civil action, he may do so by filing a complaint using the attached template. All communications with the Court by a pro se party must be sent to the Pro Se Intake Unit at 500 Pearl Street, Room 200, New York, NY 10007. Should Defendant need assistance filling out the form, he may contact the Pro Se Intake Unit at (212) 805-0175.

The Clerk of Court is respectfully directed to mail a copy of this order to Defendant.
