AFFIDAVIT OF SERVICE
I, Joshua Jurczyk, being first duly sworn on oath, depose and say the following:
I am over the age of 18, not a party to this action, and an employee of United Processing, Inc., a licensed private detective agency, license number 117-001101, appointed by the court to serve process in the above-referenced cause.
On March 23, 2021 at 1:03 PM, I served the within Alias Summons in a Civil Case and Verified . Supplemental Petition for Eviction Pursuant to 735 ILCS 5/15-1701 (h) on Unknown Occupants in; the . following manner.
Substitute Service: By leaving a copy of the Alias Summons in a Civil Case and Verified Supplemental Petition for Eviction Pursuant to 735 ILCS 5/15-1701 (h) at Unknown Occupants's usual place of abode, 6701-6709 N, Northwest Hwy, Chicago, IL 60631, with his/her, Francisco Torres, a person of his/her family, or other person residing there, over the age of 13 years who was informed of the contents of the Alias Summons in a Civil Case and Verified Supplemental Petition for Eviction Pursuant to 735 ILCS 5/15-1701(h).
On March 26, 2021, 1 mailed a copy of the Alias Summons in a Civil Case and Verified Supplemental Petition for Eviction Pursuant to 735 ILCS 5/15-1701(h) in-a sealed envelope with postage fully prepaid, addressed to Unknown Occupants at his/her usual place of abode.
Description of person process was left with:
