Granted. The district court is ordered either to rule on the merits of relator's application transferred by this Court on February 16, 1987, or to provide relator with a copy of the Uniform Application for Post-Conviction Relief and allow him to refile his claim in proper form. La. Code Crim.P. art. 926(D).
