KAYLA SMITH, JENNIFER SCALLAN AND BRENT SMITH v. WRIT NOT CONSIDERED. Relator failed to include a copy of the Motion for Extension of Time and order by this Court granting same as set forth in this Court's order of August 12, 2014. Relator also failed to comply with Rule 4-5, Uniform Rules of Louisiana Courts of Appeal, in that the writ application does not contain a copy of the petition or motion for summary judgment, in violation of Rule 4-5(C)(8). Further, relator failed to provide a notarized affidavit verifying the allegations of the application and certifying that a copy has been delivered or mailed to the respondent judge and all necessary counsel as required by Rule 4-5(A). Finally, relator failed to consecutively number all pages of its writ application, in violation of Rule 4-5(B). In order for this Court to examine the writ application, it must be presented with attachments in the manner required by the Uniform Rules of Louisiana Courts of Appeal. Supplementation of this writ application and/or an application for rehearing will not be considered. Rules 4-9 and 2-18.7, Uniform Rules of Louisiana Courts of Appeal. In the event relator seeks to file a new application with this Court, it must contain all pertinent documentation, including documentation to show the original writ application was timely, and must comply with Rule 2-12.2, Uniform Rules of Louisiana Courts of Appeal. Any new application must be filed on or before December 4, 2014, and must contain a copy of this ruling.
