IT IS ORDERED that the opinion filed herein on June 10, 2014, 226 Cal.App.4th 1209; __ Cal.Rptr.3d __ be modified as follows and the petition for rehearing is DENIED:

1. On page 21. the first line of first full paragraph [226 Cal.App.4th 1229, advance report, 2d full par., line 1], the words “Blue Cross” are deleted and replaced with the words “Blue Shield.”

2. On page 23, line six of the second full paragraph [226 Cal.App.4th 1231, advance report. 1st full par., line 7], the word “DMHC” is deleted and replaced with the words “Department of Insurance.”

There is no change in the judgment.

The petition for rehearing is denied.
