MOTION TO REQUEST APPOINTMENT OF COUNSEL

I, __________, the Plaintiff in the above-captioned action, request appointment of counsel for purposes of trial of this matter.
In support of my motion, I declare that I am not nor previously have been represented by an attorney appointed by the Court in this action, or in any other civil proceeding before this Court.
In further support of my motion, I declare that my highest level of education is ______________.
I understand that if this Motion is granted, the Court shall enter an Order placing this action before a panel of volunteer lawyers willing to represent pro se parties who are unable to afford a lawyer. I further understand that if this case is placed before the panel, this case will be stayed for a period of 90 days to permit volunteer lawyers time to review my case. I further understand that placement of the case on the panel does not guarantee that my case will be accepted by a lawyer. Unless or until a lawyer files a notice of appearance officially notifying the Court that he or she is my counsel of record, I am responsible for my case.
