Cordeiro, 18 Misc 3d 1135(A) 859 N.Y.S.2d 897 [Webster Justice Ct. 2008] this court found the accusatory instruments charging the defendant with common law and aggravated driving while intoxicated insufficient when neither the Alcohol Influence Report nor the Addendum were incorporated by reference into the supporting deposition or verified in accordance with C.P.L. § 100.30 (d).
