ORDERED that the motion for an extension of time to perfect the appeal is granted, without costs, and the time to perfect the appeal is extended to November 28, 2011, and it is further
ORDERED that the motion to accept the record on appeal is denied, without costs, on the ground that the record on appeal does not contain the notice required by CPLR 5520 (c) and the affirmation of compliance.
