MOTION DECISION

DECISION AND ORDER ON MOTION

By order dated May 27, 2021, appellant was granted poor person status and assignment of counsel. By letter dated September 21, 2021, counsel requested to be relieved of the assignment due to appellant's failure to communicate. By decision and order dated October 8, 2021, counsel was relieved of the assignment. Thereafter, by letter dated October 12, 2021, appellant was directed to perfect the appeal by November 12, 2021. Appellant failed to perfect the appeal or seek a further extension of time to do so.

Now, therefore, upon the Court's own motion, it is

ORDERED that the appeal is dismissed.
