DECISION & ORDER ON APPLICATION

Application by Nicole Phillips Cordeau, who was admitted to the Bar at a term of the Appellate Division of the Supreme Court in the Second Judicial Department on March 30, 2011, to change her name on the roll of attorneys and counselors-at-law to Nicole Phillips van de Leuv.

Upon the papers filed in support of the application, it is

ORDERED that the application is granted; and it is further,

ORDERED that the Clerk of the Court shall change the applicant's name on the roll of attorneys and counselors-at-law from Nicole Phillips Cordeau to Nicole Phillips van de Leuv, effective immediately.
