ORDER DIRECTING PARTIES TO FILE CONSENT/DECLINATION FORM

Pending before the Court is Motion to Dismiss for Lack of Jurisdiction filed by Social Security Administration. Upon review of the record in this action, the Court notes that the Consent/Declination to Proceed Before a United States Magistrate has not been filed .
