ORDER


This cause came on for consideration without oral argument on the following motion filed herein:

MOTION: AFFIDAVIT OF INDIGENCE [AND APPLICATION TO PROCEED IN FORMA PAUPERIS ] (Doc. No. 2)

THEREON it is ORDERED that the motion is DENIED without prejudice.

If Plaintiff wishes to renew his application, he must use the Court-approved form and indicate his source of subsistence. The Clerk is directed to provide a copy of the form along with this Order. In addition, Plaintiff must amend his Complaint to identify the Defendants in the style of the case and in the body of the Complaint to avoid having his application denied on grounds of ambiguity or frivolousness.
