Kathleen Kerrigan, Chief Judge.

On May 5, 2023, petitioner filed an Application for Waiver of Filing Fee and Affidavit for the Court's consideration. However, that document was not properly completed in that petitioner did not answer each part of question number one of the application. Upon due consideration, therefore, it is

ORDERED that petitioner's Application for Waiver of Filing Fee and Affidavit, filed May 5, 2023, is denied.

Petitioner is reminded that, on or before May 24, 2023, the Court's $60.00 filing fee must be paid or a properly completed Application for Waiver of Filing Fee must be submitted. Failure to do so may result in dismissal of this case for lack of jurisdiction.
