JUDGMENT


The court has dismissed this prisoner in forma pauperis compliant. A judgment of dismissal without prejudice is entered in favor of defendants. Plaintiff shall take nothing by way of his complaint.

ORDER OF DISMISSAL

This is a civil rights action filed pro se by a prisoner. The court notified plaintiff that he had not used the proper form for applying for leave to proceed in forma pauperis, had not provided a signed certificate of funds in his inmate account, and had not provided a statement showing transactions in his inmate account. He has now filed his application on the proper form and provided the signed certificate, but has not provided the statement of transactions.

Any prisoner who files a civil complaint and applies for leave to proceed in forma pauperis is required by statute to provide a statement showing transactions in his or her inmate account for the six — month period preceding the date the complaint is filed. See 28 U.S.C. § 1915(a)(2). Plaintiff has failed to provide it.

Leave to proceed in forma pauperis (docs 3 6) is DENIED. This case is DISMISSED without prejudice. See Fed.R.Civ.P. 41(b). No fee is due. Plaintiff s motion for copies (doc 2) is DENIED as moot. The clerk shall close this file.

IT IS SO ORDERED.

This matter is dismissed with prejudice.
