PER CURIAM.

Based on Appellee's concession of error, this matter is summarily reversed pursuant to Florida Rule of Appellate Procedure 9.135(b) and remanded to the trial court to hold a new disposition hearing and for further proceedings.

REVERSED and REMANDED.
