ORDER OF DISMISSAL (Doc 2)

This is a civil rights action filed pro se by a prisoner. The court notified plaintiff that his application for leave to proceed in forma pauperis ("IFP") was deficient in that several pages of the form were missing and a copy of his prisoner account statement, required by 28 U.S.C. § 1915(a)(2), was not provided. A copy of the court's form for applications to proceed in forma pauperis was provided with the notice, along with a return envelope. Plaintiff was informed that if he did not either pay the fee or file a complete application and a copy of his prisoner trust account covering the specified period the case would be dismissed. No response has been received.

Leave to proceed in forma pauperis (doc 2) is DENTED. This case is DISMISSED without prejudice. See Fed.R.Civ.P.41(b). No fee is due. The clerk shall close this file. IT IS SO ORDERED.
