DECISION AND ENTRY FINDING DEFENDANT IN VIOLATION OF

SUPERVISED RELEASE, REVOKING SAME AND IMPOSING SENTENCE,

WITH THREE YEAR PERIOD OF RE-IMPOSED SUPERVISED RELEASE

TO FOLLOW, SUBJECT TO CERTAIN CREDITS; CONDITIONS OF

REIMPOSED PERIOD OF SUPERVISED RELEASE SET FORTH; RIGHT OF

APPEAL EXPLAINED AND UNDERSTOOD; TERMINATION ENTRY

On August 5, 2013, the Defendant, having previously been found in violation of her supervised release which began January 26, 2011, appeared in open Court for final disposition.

Pursuant to the record made in open Court on the aforesaid August 5, 2013, the Defendant's supervised release was revoked and she was remanded to the custody of the Attorney General of the United States, the Bureau of Prisons, for a period of time served, with the original three year period of supervised release reimposed, with credit given for all time spent in confinement, on her revocation matter, until the date of her release. As conditions of her re-imposed period of supervised release, Defendant is to follow all the rules and regulations of the Probation Department, work toward and complete all undischarged conditions of supervision previously imposed on the aforesaid January 26, 2011, and, further, comply with each and every aspect of her final Supervision Plan, dated August 2, 2013 (Doc. #120), to which she agreed in open Court on the aforesaid August 5, 2013. The gist of said final Supervision Plan is as follows:

1. Defendant will enter and successfully complete the inpatient program at Women's Recovery Center, located in Xenia, Ohio, and follow all recommended aftercare.

2. Defendant will remain in the custody of the United States Marshals Service until a bed becomes available at the aforesaid Women's Recovery Center.

3. Upon completion or termination of the Women's Recovery Program, Defendant will be living at 103 Nimitz Drive, Dayton, Ohio 45431, where she will be placed on location monitoring at the discretion of the Probation Officer until further order of the Court. The Probation Officer is to notify the Court as to the date when Ms. Back either completes or is terminated from the Women's Recovery Program, in order that a brief conference call may be established.

Following the above, the Defendant was orally explained her right of appeal, and she indicated an understanding of same.

The captioned cause is hereby ordered terminated upon the docket records of the United States District Court for the Southern District of Ohio, Western Division, at Dayton.
