ORDER

Upon consideration of appellant's non-conforming consent to collection of fees from trust account, it is.

ORDERED that appellant submit to this court a signed, unconditional consent to collection of fees form within 30 days of the date of this order. See 28 U.S.C. § 1915(b). Appellant's failure to comply with this order will result in dismissal of the appeal for lack of prosecution.See D.C. Cir. Rule 38.

The Clerk is directed to send a copy of this order, and a copy of the consent to collection of fees form, to appellant by whatever means necessary to ensure receipt.
