Dear Counsel:
The Court has received Ms. Saurman's letter dated November 19, 2009. Consistent with the Court's opinion dated September 21, 2009, the plaintiff should be provided with all forms the DME physician wants her to complete before the day of the DME, which is December 7, 2009. The defendant should insure that the forms are provided to the plaintiff on or before December 2, 2009.
