FINAL JUDGMENT


The Court having issued its Order on Renewed Motions for Summary Judgment and Order Granting Plaintiff's Motion for Leave to File a Supplemental Memorandum and Denying Plaintiff's Request for Discovery on December 23, 2004 hereby ORDERS, ADJUDGES AND DECREES that Judgment is entered in favor of Plaintiff, Kenneth R. Omasta, and against Defendant Reliance Life Insurance Company as follows:

1. Defendant Reliance Standard Life Insurance Company shall pay Plaintiff long-term disability benefits under its Long Term Disability (LTD) Insurance Policy;

2. Benefits owed for June 2000 through December 31, 2004 are $269,844.91;

3. Pre-judgment interest at 10% per annum from June 18, 2000 through December 31, 2004 is $62,378.61;

4. For a total judgment of $332,223.52, together with post-judgment interest calculated at the statutory rate as per 28 U.S.C. § 1961(a); and

5. Monthly benefits of $4,777.23 from the nineteenth date of each month, commencing December 19, 2004, through the eighteenth day of the following month, together with post-judgment interest.
