On May 10, 2022, the parties filed a settlement stipulation and a proposed stipulated decision. Although the settlement stipulation states that a Form 3623 (IRS Statement of Account) is attached thereto, no Form 3623 is attached.

Upon due consideration, it is

ORDERED that the parties' settlement stipulation, filed May 10, 2022, is stricken from the Court's record. It is further

ORDERED that, on or before June 15, 2022, the parties shall file a revised settlement stipulation.
