In Re: Diana Lewis and Brian Lewis, applying for supervisory writs, 19th Judicial District Court, Parish of East Baton Rouge, No. 549358.

BEFORE: KUHN, PETTIGREW AND McDONALD, JJ.

WRIT NOT CONSIDERED. Relators have failed to comply with Rules 4-2, 4-3, and 4-5C, Uniform Rules of Louisiana Courts of Appeal. The writ application does not contain an affidavit verifying the allegations of the application and certifying that a copy has been delivered or mailed to the respondent judge and to opposing counsel, a violation of Rule 4-5 (A) . The writ application also does not contain a Notice of Intent with Request for Return Date, violations of Rules 4-2 and 4-3; therefore, there is no documentation that relators filed a notice to seek writs with the district court within the 30-day time limit set by Rules 4-2 and 4-3. Most importantly, relators did not include a copy of the judgment or ruling complained of in violation of Uniform Rules of Louisiana Courts of Appeal, Rule 4-5 (C) (6) and (10} .

Supplementation of this writ application and/or an application for rehearing will not be considered. Rules 4-9 and 2-18.7, Uniform Rules of Louisiana Courts of Appeal.
