DECISION AND ORDER ON MOTION

Motion for permission to proceed as a poor person and for further relief.

Upon the papers filed in support of the motion, and no papers having been filed in opposition thereto, it is

ORDERED that the motion is granted, without costs, only to the extent that the filing fees required by CPLR 8022 are waived. Pursuant to 22 NYCRR 850.9 (b) (3) and 22 NYCRR 1250.9 (a), (e), the appeal may be perfected upon an original and five copies of a reproduced full record and an original and five copies of the appellant's brief, or one copy of the complete record on appeal and an original and five copies of the appellant's brief and appendix, and an additional unbound hard copy of any document filed.
