ORDER

Maurice B. Foley Chief Judge

On December 14, 2021, petitioner filed an Application for Waiver of Filing Fee. However, that application is incomplete: Petitioner has not checked the boxes indicating whether or not he receives income from five of the six possible sources listed on the application. Additionally, the caption on the application does not match the caption of this case.

Accordingly, as petitioner has not established that he lacks the ability to pay the Court's $60.00 filing fee, it is

ORDERED that petitioner's application for waiver of filing fee is denied. It is further

ORDERED that, on or before January 27, 2022, petitioner shall pay the Court's $60.00 filing fee for this case. Failure to comply with this Order may result in the dismissal of this case or other appropriate action by the Court. It is further

ORDERED that the Clerk of the Court shall attach to the copy of the Order served on petitioner information regarding payment of the Court's filing fee.
