The uncontested report of the referee is approved and respondent is disbarred, effective immediately.

Respondent shall pay restitution in the amount of $4,955.00 to The Neurological Group, plus any accrued interest or other late fees due to them; and in the amount of $8,107.00 to Baptist Hospital, plus any accrued interest or other late fees due to them; and in the amount of $3,000.00 to Donald and June C. Wilson, under the terms and conditions set forth in the report.

Judgment is entered for The Florida Bar, 651 East Jefferson Street, Tallahassee, Florida 32399-2300, for recovery of costs from Maldrick Elaine Bright in the amount of $4,169.79, for which sum let execution issue.

Not final until time expires to file motion for rehearing, and if filed, determined. The filing of a motion for rehearing shall not alter the effective date of this disbarment.
